The Arora Group, Inc. no longer automatically mails paper copies of Form 1095-C (Employer-Provided Health Insurance Offer and Coverage). This form is used for IRS tax filing purposes to verify that health insurance coverage was offered during the 2025 calendar year.
How to Request Your Form:
Current or former employees may request a copy of their 2025 Form 1095-C by using one of the following methods:

Delivery Timeline:
Once a request is received, Arora will furnish the form within 30 days. Paper copies will be mailed by January 31, 2026, for all requests received prior to that date.
Electronic vs. Paper Delivery
Electronic Delivery: Employees who have previously provided consent for electronic communications may access their Form 1095-C through UKG:
- My HR | Forms | Government Forms | 1095-C
- Use the Selected Year dropdown to choose 2025
- Click Apply
- Select the pencil icon next to 2025 to view the form
Paper Delivery: Employees who have not approved electronic communications or who specifically request a paper copy will receive Form 1095-C via first-class mail to the last known address.
